About the Role

We are seeking a compassionate and experienced Community Registered Nurse to join our growing team. This role involves delivering clinical nursing services to NDIS participants and older Australians living in the community, ensuring safe, high-quality care that promotes health, wellbeing, independence, and quality of life.

The successful applicant will work collaboratively with participants, families, support workers, allied health professionals, and external healthcare providers to coordinate and deliver person-centred care.

Key Responsibilities

  • Conduct nursing assessments and develop care plans.

  • Perform clinical procedures including wound care, catheter care, medication administration, diabetes management, PEG feeding, Palliative Care and continence support.

  • Monitor participant health and identify changes in clinical status.

  • Support participants with complex health needs and chronic disease management.

  • Develop and review clinical documentation and progress notes.

  • Provide clinical guidance, education, and support to disability support workers and care staff.

  • Liaise with GPs, hospitals, specialists, allied health professionals, and family members.

  • Participate in incident management, risk assessments, and quality improvement activities.

  • Ensure compliance with NDIS Practice Standards, Aged Care Quality Standards, and organisational policies.

  • Working on an on-call rostered system

Essential Requirements

  • Current registration with AHPRA as a Registered Nurse.

  • Minimum 3 years' post-registration nursing experience.

  • Experience in disability, community nursing, aged care, or complex care environments.

  • Current First Aid and CPR certification.

  • Current NDIS Worker Screening Clearance.

  • Current Working with Children Check (or ability to obtain).

  • Current Australian Driver Licence and reliable vehicle.

  • Strong clinical assessment and care planning skills.

  • Excellent communication, documentation, and time-management skills.

Desirable Qualifications and Experience

  • Experience supporting NDIS participants with complex disabilities.

  • Knowledge of Positive Behaviour Support and restrictive practices requirements.

  • Experience with aged care services and Home Care Packages.

  • Competency in wound management, medication management, and chronic disease care.

  • Experience using electronic clinical record systems.
